Camp Yedid Contract

I hereby authorize and permit Camp Yedid and/or its representatives to provide emergency medical care and treatment of my minor child, including transport of my child to the nearest hospital or emergency clinic, in the event of said emergency and I will personally be responsible and liable for the necessary and reasonable expenses and charges incurred by said medical treatment. Camp Yedid shall make a reasonable effort to contact or telephone me, immediately. I hereby authorize and permit Camp Yedid to take my child off camp grounds to go on trips organized as part of the camp program. In addition, my child may participate in any activity organized by Camp, and I assume the inherent risk of all activities and camp programs. I will hold Camp Yedid and/or its representatives harmless in the events of injury or property damage or loss as a result of such activities. I hereby give permission to Camp Yedid to use all pictures taken during the summer for publicity purposes. I understand that by signing this application and by sending my child to Camp Yedid that all unclaimed items will be assumed “hefker” (ownerless) the day after camp ends, and will be disposed of at our discretion. I understand that Camp Yedid reserves the right to remove or suspend any child from camp. Refunds will be made accordingly. The camp has the right not to accept a child at any given time; signing this form does not ensure registration. No camp fees will be deducted before acceptance is made.
